上海市艾滋病防治工作中医患沟通能力障碍及其干预方案效果的研究

Doctor-Patient Communication Barriers in AIDS Prevention and Treatment and the Effect of Intervention Programs, Shanghai

  • 摘要: 目的 了解在艾滋病防治工作中医护人员与艾滋病感染者及病人(people living with HIV/AIDS,PHA)之间沟通能力所存在的障碍,评估干预方案的效果。

    方法 基线资料采用个人深入访谈和小组访谈,访谈了50名PHA以及46名包括政策决策者、服务提供者等在内的相关人员,定性调查资料采用N-Vivo 8.0进行编码和分析。干预方案采用个案模拟和角色扮演等形式,并对培训效果采用问卷评估,数据采用SPSS 11.5进行频数分析。

    结果 基线研究发现PHA在医疗服务过程中所存在的主要障碍有医疗服务的可及性不够、PHA的心理问题、职业暴露相关问题;干预研究达到了改善医护人员与PHA之间沟通障碍的预期目标。

    结论 互动式干预培训是一种很好的交流方式,本干预研究达到了减少和消除医患之间沟通能力障碍的目标,也为其他服务人群与服务对象之间的沟通能力障碍培训提供了方向。

     

    Abstract: Objective To understand the communication barriers between people living with HIV/AIDS(PHA) and health professionals in AIDS prevention and treatment, and to assess the effect of intervention programs.

    Methods Baseline data were collected from 50 PHA and 46 relevant personnel (policy makers, service providers, non-governmental organizations and patients' families) using in-depth interviews and focus group discussions. N-Vivo 8.0 software was used for coding and data analysis. Case simulations and role-playing were taken in the intervention, the intervention effect was assessed by questionnaire and the data was analyzed by SPSS 11.5.

    Results The baseline findings showed that the main obstacles for PHA in the health care services were the poor accessibility to health care services, PHA mental health problems and occupational exposures. The intervention program achieved the goal of improving doctor-patient communication.

    Conclusion The interaction intervention is an effective communication method, through which the goal of eliminating the communication barriers between PHA and health professionals was achieved. It is also a good example for other communication trainings.
