Characteristics and trends of accidental fall mortality among residents in Pudong New Area of Shanghai, 2002-2016

  • 摘要: 目的 了解2002-2016年浦东新区居民意外跌落死亡的流行病学特点及其变化趋势,为制定相应的干预措施提供依据。

    方法 收集2002-2016年上海市浦东新区居民意外跌落死亡资料,进行流行病学分析,计算意外跌落的粗死亡率、中国人口标化死亡率、平均年变化率(AAPC)及减寿指标。标化死亡率的比较采用Mantel-Haenszel检验,运用Joinpoint 4.0.4对死亡率变化趋势进行分析。

    结果 2002-2016年浦东新区意外跌落年均粗死亡率为13.28/10万。标化死亡率呈现逐年下降的趋势(AAPC=-3.85%,Z=-5.506,P < 0.001);女性意外跌落标化死亡率(6.80/10万)低于男性(7.07/10万)(Z=-5.79,P < 0.001)。伤害部位以颅脑与下肢为主,占死亡总数的75.72%。浦东新区意外跌落死亡潜在减寿年数为18 415人年,潜在减寿率为0.46‰,平均减寿年数为3.44年/人。

    结论 浦东新区意外跌落死亡率呈现逐年下降的趋势,高处跌落死亡值得关注,不同人群意外跌落死亡情况存在差异。

     

    Abstract: Objective To assess the epidemiological characteristics and trends of accidental fall mortality among residents in Pudong New Area from 2002 to 2016, so as to provide a reference for developing relevant intervention measures.

    Methods Data on accidental fall mortality among residents in Pudong New Area from 2002 to 2016 were retrieved from the database of the residential death cause surveillance program. Crude mortality rate, age-standardized mortality rate by China standard population, average annual percent change (AAPC), life lost related indicators due to accidental fall were calculated. The agestandardized mortality rate was estimated by Mantel-Haenszel test, and the mortality trend was tested with Joinpoint 4.0.4.

    Results The annual average crude mortality rate for accidental fall was 13.28/105 in Pudong New Area during 2002-2016. During the selected 15 years, the age-standardized mortality rate for accidental fall was declining (AAPC=-3.85%, Z=-5.506, P < 0.001). Females (6.80/105) showed a lower accidental fall age-standardized mortality rate than males (7.07/105) (Z=-5.79, P < 0.001). Head injuries and lower limb injuries accounted for 75.72% of total mortality of accidental fall. The potential years of life lost, potential years of life lost rate, and average years of life lost due to accidental fall were 18415 person-years, 0.46‰, and 3.44 years/person, respectively.

    Conclusion The accidental fall mortality in Pudong New Area during 2002-2016 shows a declining trend. More attention should be paid to the mortality caused by falling from a height. Populations with different characteristics show varied accidental fall mortality, which requires differential intervention measures.
